Community-based transition programs
Services are developed to transition students from a more self contained setting back into a public school building. Services include one on one support for the achievement of behavioral and academic goals. Transportation is provided. A student may attend both settings during the day with a gradual increase in participation at the school. The student is gradually introduced to the staff, school building and schedule. The student works with a special education case manager who supervises the transition. The pace of the transition is dictated by the student's educational team.
